Admission Information

Welcome to St. Stanislaus Kostka School. When you enter St. Stan’s, you become a part of our family. Our students do not enter the doors alone; their family enters with them. Families, students, and the school family of faculty and staff commit themselves to the mission and vision of the school. We all work in partnership in achieving and continuing the excellence of St. Stanislaus Kostka School.

Late registration is for the 2012-2013 school year is available with openings in PS-7th grade. Registration is ongoing until all seats are filled. After looking over our website, you believe St. Stan’s is the place for you and your child, take these steps to become a Wildcat!

1. Family Interview - Call our school at 773-278-4560 to schedule a tour of our building and meet with Ms. Hill, our principal. Please bring your child/ren’s report cards for the past two years and any standardized test scores.
2. Student Assessment and Shadow Day - Transfer students in grades 1-8 will take a math and reading inventory. This can be combined with a shadow day on a mutually determined date between the family and St. Stan’s.
3. Admissions decision is made - Families will be notified within two days via phone regarding admissions decision.
4. Completed registration packet - To reserve your child’s spot, the registration packet must be completely filled out and the $180 registration fee attached. The following documentation is also needed:
   • Your child’s original birth certificate (will be photocopied and returned)
   • Copy of baptismal certificate (if applicable)
   • Parent Questionnaire
   • Request for transfer records from previous school
   • Immunization record is due before first day of school.

For students entering PK and Kindergarten, the following age requirements must be met:

• PK 3—Age 3 by September 1st
• PK 4—Age 4 by September 1st
• Kindergarten—Age 5 by September 1st

Then the process can begin!

2. Classroom visit - Children entering PK or Kinder need to schedule a half hour block of time, preferably in the morning, to spend in their respective classroom. Parents are welcome to join their child, but if possible we would like the child to be on their own for at least 15 minutes. It isn’t always possible, but we try to give it a go!
3. Admissions decision is made - Families will be notified within two days via phone regarding admissions decision.
4. Completed registration packet - To reserve your child’s spot, the registration packet must be completely filled out and the $150 registration fee attached. The following documentation is also needed:
   • Your child’s original birth certificate (will be photocopied and returned)
   • Copy of baptismal certificate (if applicable)
